Accident Insurance Coverage

Accidents and injuries can happen at any place at any time. As your family's most important asset, it is important to protect yourself and make sure you can bounce back from whatever life may throw at you. The accident insurance plan offers Off-the-Job coverage for accidents. The annual wellness benefit covers an annual health screening test such as mammography, colonoscopy and cholesterol test. The Accident Insurance Plan pays in addition to any other coverage and coverage cannot be denied due to past medical history and there are no medical questions.

Individual and Family coverage is available for this plan.

The Accident Insurance Plan includes the following benefits:
  • Accident Emergency Treatment Benefit
      Provides for physician treatment and X-rays in a hospital or doctor's office within 96 hours of the accident.
  • Major Diagnostic Examination Benefit
      Provides for one CT Scan, MRI, or EEG completed within 90 days of the accident.
  • Dislocation Benefit
      Reduces the cost of anesthesia required as a result of joint dislocation (i.e. knee, shoulder, hip, etc...).
  • Fractures Benefit
      Pays for repair of a fracture sustained in an accident; includes chips, simple, radial and compound fractures.
  • Accident Follow-Up Treatment Benefit
      Covers a maximum of three (3) follow-up visits per accident.
  • Initial Accident Hospitalization Benefit
      Pays for the first hospital admission due to an accident. Benefit also covers the first Intensive Care Unit admission due to an accident.
  • Ambulance Benefit
      Pays for ambulance transportation by a licensed ambulance service to the nearest hospital for treatment.
  • Wellness Benefit
      After a 30-day waiting period, benefit is payable per calendar year for one annual health screening test listed for the primary member and one test for a covered spouse; and includes:

    • Blood test for triglycerides
    • Bone marrow testing
    • Breast ultrasound
    • CA 125 (blood test for ovarian cancer)
    • CA 15-3 (blood test for breast cancer)
    • CEA (blood test for colon cancer)
    • Chest X-ray
    • Colonoscopy
    • Fasting blood glucose test
    • Flexible sigmoidoscopy
    • Hemocult stool analysis
    • Mammography
    • Pap test
    • PSA (blood test for prostate cancer)
    • Serum cholesterol test to determine HDL/LDL level
    • Serum Protein Electrophoresis(blood test for myeloma)
    • Stress test on a bicycle or treadmill
    • Thermography
